We are seeking a dedicated and skilled Physical Therapist to join our dynamic healthcare team. The ideal candidate will possess a strong foundation in physiotherapy practices, with experience in inpatient, outpatient, and home health settings. This role involves developing personalized care plans, delivering high-quality patient care, and collaborating with multidisciplinary teams to promote recovery and functional independence across diverse populations.
*Responsibilities*
* Assess patients' physical conditions by reviewing medical histories, conducting examinations, and utilizing relevant physiology knowledge.
* Develop and implement individualized care plans tailored to each patient’s needs, including discharge planning for hospital or community transitions.
* Administer therapeutic interventions such as manual therapy, trigger point therapy, and exercise programs to improve mobility, strength, and function.
* Monitor patient progress regularly, adjusting treatment strategies to optimize outcomes while ensuring adherence to clinical protocols.
* Document patient interactions accurately within electronic medical record (EMR) systems and communicate effectively with healthcare teams.
* Provide specialized care for pediatric, geriatric, orthopedic, and neurological patients in various settings including inpatient units, outpatient clinics, home health visits, and hospital environments.
* Educate patients and their families on physiotherapy techniques, injury prevention strategies, and self-care practices to foster long-term wellness.
*Requirements*
* Valid licensure or certification as a licensed Physical Therapist in the relevant jurisdiction.
* Proven experience in acute care hospitals, outpatient clinics, or home & community care environments with familiarity in inpatient and outpatient settings.
* Strong knowledge of anatomy, physiology, medical terminology, and rehabilitation techniques including trigger point therapy.
* Experience with discharge planning processes and developing comprehensive care plans for diverse patient populations.
* Proficiency with electronic medical records (EMR) systems for documentation and communication purposes.
* Excellent patient care skills combined with the ability to work collaboratively within multidisciplinary teams.
* Knowledge of physiotherapy practices across pediatrics, geriatrics, orthopedics, and geriatrics is highly desirable. Pay: $93,600.00 - $117,000.00 per year
