All Care is seeking a Speech Therapist to evaluate, diagnose, and treat patients with speech, language, cognitive-communication, and swallowing disorders in a home-based setting. The Speech Therapist works collaboratively with an interdisciplinary team to improve patient function, safety, and quality of life while supporting independence in the home environment.

Work Location: On the road in assigned territory
*Essential Duties & Responsibilities:*
* Perform comprehensive speech, language, cognitive, and swallowing evaluations in patients’ homes
* Develop and implement individualized treatment plans based on patient needs and goals
* Provide therapeutic interventions for communication disorders, cognitive deficits, and dysphagia
* Educate patients, families, and caregivers on therapy techniques, home exercises, and safety strategies
* Monitor patient progress and modify treatment plans as needed
* Collaborate with physicians, nurses, physical therapists, occupational therapists, and other care team members
* Recommend adaptive equipment or strategies to enhance communication and swallowing safety
* Ensure compliance with home care regulations, policies, and documentation standards
* Complete accurate and timely clinical documentation in EMR systems
* Participate in case conferences and interdisciplinary team meetings
*Qualifications:*
* Valid driver’s license and reliable transportation
* Graduate of an accredited speech language pathologist master’s degree program
* Active Massachusetts SLP license
* One year of experience in a health care setting
* Proficiency in using electronic health record (EHR) systems for documentation and communication.
